This outpatient opportunity offers a senior-focused patient population, interdisciplinary support, academic teaching opportunities, and the ability to build long-term patient relationships in a collaborative, team-based care model.

Practice Highlights / Qualifications- Board Certified or Board Eligible in Internal Medicine or Family Medicine required
- Minimum three years of experience in senior care, primary care, or post-acute care preferred
- Outpatient-only practice focused on value-based, senior-centered care
- Collaborate with APPs, RN care coordinators, social workers, and behavioral health professionals
- Opportunity to supervise advanced practice providers
- Experience managing complex patient populations and quality-based outcomes preferred
- Familiarity with value-based care, risk adjustment, and senior-heavy patient panels highly desired
