You will educate patients and caregivers on home exercise programs, energy conservation, fall prevention, and mobility strategies tailored to each unique home environment.A typical day consists of traveling to several patient homes within a defined territory around Bluefield, performing initial evaluations and follow-up treatment visits, collaborating with the broader home health team, and completing electronic documentation. Visit volumes are designed to be clinically appropriate and manageable, allowing you to maintain high-quality, personalized care without sacrificing thorough assessment and education. While caseloads and visit numbers can vary, the focus remains on safe, effective, and functional outcomes for each individual.You will work closely with nurses, occupational therapists, speech therapists, social workers, and referring providers to coordinate care plans and ensure seamless communication.
