Overview:
Our team provides comprehensive, patient-centered care directly in the community setting. This role supports patients through skilled nursing visits, ensuring continuity of care from admission through ongoing treatment. Our team is focused on delivering high-quality outcomes while maintaining strong clinical independence in the field.

Position Offers:

• Schedule: Flex Monday Friday
• Full-time, permanent employment
• Compensation starting at $80,000+ (DOE)

Key Responsibilities:

• Conduct in-home skilled nursing visits for assigned patients
• Complete start-of-care assessments and ongoing evaluations
• Administer medications and prescribed treatments
• Document patient care accurately and in a timely manner
• Review, update, and sign off on clinical charts
• Monitor patient status and report changes to care teams
• Coordinate care plans with interdisciplinary providers
• Ensure compliance with care standards and protocols

Qualifications:

• Active Registered Nurse (RN) license in New Mexico or compact state
• Minimum of 1 year of RN experience
• Experience in clinical or acute care setting preferred
• Strong documentation and clinical assessment skills
• Valid driver s license and reliable transportation
