Key Responsibilities
- Conduct thorough physical assessments of patients in their home environments to identify individual needs, goals, and barriers to mobility.
- Develop and implement personalized treatment plans based on assessment findings and patient preferences, ensuring they adhere to best practices and evidence-based approaches.
- Utilize various therapeutic modalities and techniques to improve patients' physical abilities, such as therapeutic exercises, functional training, and pain management strategies.
- Collaborate with other healthcare professionals, including Occupational Therapists and Rehabilitation Medicine physicians, to create integrated care plans that address all aspects of a patient's health.
- Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes, documenting each session accurately and thoroughly in electronic health record (EHR) systems.
- Provide education to patients and their families about therapeutic exercises, home safety, and injury prevention, fostering independence and health awareness.
- Participate in regular team meetings and case discussions to address patient care strategies and share insights regarding treatment methodologies and patient responses.
- Ensure compliance with all legal, ethical, and professional standards related to patient care.

Qualifications
- Bachelor's degree in Physical Therapy from an accredited institution.
- Current state licensure to practice as a Physical Therapist in California.

Compensation and Benefits
